A summary statement only shows department totals. The scrub needs the version with a code and amount on every line. Ask the hospital’s billing office for a complete itemized statement.
“A complete itemized statement with every charge as its own line: date of service, CPT/HCPCS or revenue code, description, quantity, and amount.” Ask them to note on the account that it’s under review.
